Follow directions in the guidelines carefully. This filled out form must be emailed with your contest entry. The pdf entry form includes: 30 pages of your script, one page synopsis & cover sheet.
Remember, you must email a PDF copy of your contest entry pages as well. Don't forget! This is the entry form only.
Your entry is not valid until we receive your fee.
